Roughly half to two-thirds of women report cognitive symptoms during the menopause transition (Maki & Jaff, Climacteric 2022). And thanks to a landmark 6-year longitudinal study — the Study of Women's Health Across the Nation (SWAN), tracking 1,903 midlife women — we now know that perimenopause produces a measurable, transient dip in processing speed and verbal memory that isn't fully explained by poor sleep, hot flashes, or mood changes (Greendale et al., Neurology 2009). Tier 1: Fix the Deficiency First

If your bloodwork shows low ferritin, low B12, or low vitamin D, correcting that deficiency may resolve cognitive symptoms entirely — because the "brain fog" was a deficiency symptom, not a hormonal one.

Iron: The challenge is tolerability. A meta-analysis found that ferrous sulfate more than doubled the odds of GI side effects compared to placebo, with roughly a third of users affected (Tolkien et al., PLOS ONE 2015). That's a leading reason people stop taking iron. Lower-dose formats, iron bisglycinate, or alternate-day dosing may help — work with your doctor on the right form and dose.

B12: Both sublingual and oral B12 effectively restore serum levels — a 2025 meta-analysis of 16 studies with 6,098 participants found both routes are as effective as intramuscular injection (Frontiers in Pharmacology 2025). The sublingual route may be worth considering if you take acid-suppressing medications or metformin, which impair GI absorption. But this is a format convenience argument, not a bioavailability multiplier.

Vitamin D: High rates of insufficiency in perimenopausal women are well-documented. Supplementation improves levels, and some observational data link adequate vitamin D to better mood and cognitive function — but RCTs showing cognitive improvement from supplementation are limited mainly to people who were deficient.

Tier 2: Sleep Support (the Indirect Pathway)

Poor sleep is both a symptom and a driver of perimenopause brain fog. Hot flashes, night sweats, and disrupted sleep architecture all degrade cognitive performance the next day. Supporting sleep quality is a legitimate — if indirect — approach to brain fog.

Magnesium: No RCTs for cognition in perimenopausal women specifically, but magnesium glycinate has evidence supporting sleep quality in older adults (Abbasi et al., J Res Med Sci 2012). Better sleep → less fog. It's an honest, modest claim.

Tier 3: Emerging Evidence (Be Skeptical)

Soy isoflavones: The best-studied supplement for cognition in this population. A meta-analysis of 16 RCTs (n=1,386) found modest improvement in overall cognitive function and memory — but no significant effect on executive function, processing speed, or attention (Cheng et al., Menopause 2015). A larger long-term trial (Henderson et al., Neurology 2012) found no benefit. Call it a mixed-but-modestly-positive signal.

Omega-3s: Higher EPA/DHA intake is associated with better brain function in observational studies, but RCT evidence in menopausal women specifically is sparse. A 2025 narrative review called for trials that don't yet exist (PMC12209554). "Supports normal brain function" is defensible. "Improves memory" is not.

Lion's mane: One small RCT of 30 menopausal women found 2 g/day for 4 weeks significantly reduced depression and anxiety scores — but the outcome measured was mood, not cognition (Nagano et al., Biomed Res 2010). The Format Question: Does Delivery Method Matter Here?

The honest recommendation for perimenopausal brain fog support is a combination of formats: some nutrients fit a strip, most don't. Anyone selling you a single product as a "complete menopause brain fog solution" is selling you a story.

What I'd Actually Recommend (The Routine I'd Put My Mom On)

Step 1: Get the bloodwork. TSH, ferritin, CBC, B12, vitamin D. Non-negotiable. Every other recommendation depends on what those numbers say.

Step 2: Fix any deficiency. If ferritin is low, work with your doctor on an iron protocol.
