A friend of mine started semaglutide last year. The weight came off, and so did something she hadn't expected — what she called "the noise about eating" simply stopped. She was happy with the medication and still is.
What prompted this article was a follow-up appointment where her endocrinologist asked, gently, whether she'd been doing any weight-bearing exercise. The answer was no. Nothing about that appointment was alarming. It was a doctor doing their job, raising bone health as part of monitoring someone through significant weight loss, and making a plan with her.
I want to be clear about what this article is and isn't, before anything else. It is a plain-language summary of what published research reports about rapid weight loss and bone mineral density. It is not medical advice, it is not a treatment guide, and it is not a pitch. I run a supplement company. I am not going to use a prescription medication's side-effect profile to sell you a strip, and if you came here hoping for a product that addresses this, I don't have one and neither does anybody else. If you are taking or considering a GLP-1 medication, every decision described below belongs with your prescriber.
What GLP-1 agonists do, briefly
GLP-1 (glucagon-like peptide-1) receptor agonists are a class of prescription medications originally developed for type 2 diabetes. Semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) are the most discussed examples. They mimic a gut hormone that slows gastric emptying, increases satiety and improves insulin response. At the doses approved for chronic weight management, the clinical trials have reported substantial and durable weight loss.
Their benefits in metabolic markers are well documented and their safety profile is reasonably well characterised through regulatory review. The question researchers have been examining is a narrower one: what happens to bone density when an adult loses a large amount of weight over a relatively short period.
What the research actually says about bone density
The important framing, and the one most consumer coverage skips: the signal researchers describe is not specific to these medications. It appears in the broader literature on rapid weight loss from any cause.
The bariatric surgery literature. The most-studied form of large, medically supervised weight loss is bariatric surgery, and that bone-density literature goes back decades. Published studies have documented measurable declines in bone mineral density at the hip and spine in the period following surgery, with some longer-term cohorts reporting higher fracture rates. This is established enough that post-surgical bone monitoring is a routine part of care.
Weight-loss medication data. Bone density was not a primary endpoint in the major weight-management trials, so most of what exists comes from imaging substudies and secondary analyses. Researchers examining these have generally described findings consistent with the wider rapid-weight-loss picture rather than something unique to the drug class. This is an area where the evidence is still developing and where responsible summaries — including this one — should avoid overstating what any single analysis shows.
Proposed mechanisms. The leading explanations in the literature are not "these drugs attack bone." They centre on the fact that losing a large amount of body mass reduces the mechanical load the skeleton carries, and that eating substantially less can mean taking in less of the nutrients and protein that support bone and muscle. Bone remodels in response to load and available building blocks.
The fair reading: significant, rapid weight loss is associated with measurable changes in bone density in some people, and the association is not unique to GLP-1 medications. Whether those changes translate into higher long-term fracture risk in this population remains an open research question. Anyone who tells you it is settled — in either direction — is ahead of the evidence.
What your doctor is likely to focus on
I'm listing this so you know what a normal, competent conversation looks like — not as instructions to follow on your own. Clinical guidance on weight loss and bone health tends to converge on:
- Baseline imaging where indicated. A DEXA scan before or early in significant weight loss, particularly for postmenopausal women, older men, people with prior fractures, or anyone with existing osteoporosis risk factors.
- Resistance training. Bone responds to mechanical load, and strength work appears consistently in weight-management guidance for protecting bone and lean mass. The specific prescription should come from someone who knows your situation.
- Adequate protein intake during active weight loss, to protect lean mass. Your clinician or a registered dietitian can set a target appropriate to your body and your medication.
- Adequate calcium and vitamin D status. Assessed against your bloodwork and your diet — not guessed at from an article.
- Follow-up imaging for patients with baseline risk, to see the trajectory rather than a single snapshot.

Where nutrition fits in the picture
Nutrition during weight loss is a genuine topic, and it's one clinicians and dietitians handle routinely. If you're eating considerably less food, you may be taking in less of several nutrients, and that's worth attention. Whether that attention takes the form of dietary change, a specific supplement, a specific dose, or nothing at all depends on your bloodwork, your diet, your other medications and your risk profile.
Which is exactly why I'm not going to prescribe anything here. A supplement company is not in a position to tell you what your nutritional gaps are during medical treatment, and any brand that presents itself as the answer to a medication's effects is doing something I think is indefensible. The person who can answer this is the person who ordered your labs.
Why we're not positioning a product in this article
The earlier version of this article had a section here pitching our Bone Support strip as fitting alongside GLP-1 treatment. We've removed it, and I'd rather explain why than quietly delete it.
Positioning a dietary supplement as support for the effects of a prescription medication implies it does something we have no evidence it does. We have not run a trial on our finished product. There is no research showing that any supplement prevents, reverses or offsets bone-density change during medication-assisted weight loss. Writing around that with careful hedging would still have left the impression that buying a strip was a sensible response to reading this page. It isn't.
For completeness and nothing more: we do make a product called Bone Support. It contains vitamin D3 (cholecalciferol) 2000 IU and vitamin K2 (menaquinone) 120 mcg, raspberry flavour, thirty strips per tin at $29.99. It is not vegan — the D3 is derived from lanolin. D3 and K2 are fat-soluble and are absorbed in the gut with dietary fat, which is true whether they arrive as a strip, a capsule or a drop; controlled trials have not shown sublingual D3 to be superior to oral D3. We sell it because some people can't swallow capsules, full stop. If you're on a GLP-1 medication, whether you should take any vitamin D or K product — and at what dose — is a question for your prescriber, who may also want to check your levels first.
What's not appropriate to promise
Explicitly, because the wellness internet is loose with this and I don't want this page drifting into it:
- No supplement has been shown to prevent or reverse bone-density changes associated with weight loss on these medications. The research isn't there. Anyone telling you otherwise is selling something.
- "Bone support" supplements are not a treatment for osteoporosis. Osteoporosis is a diagnosed medical condition with prescription treatments that have real fracture-prevention evidence behind them. That's a doctor's territory.
- Never adjust or stop a prescription based on an article. Including this one.
- If you're already on bone medication — bisphosphonates, denosumab, romosozumab — adding any supplement is a conversation with your prescriber, not a unilateral decision. Interactions and duplicate intake are real considerations.
- Resistance training, protein and adequate calcium have far better evidence behind them than any vitamin product. Don't let a supplement stand in for those.
Frequently asked questions
Should I stop my GLP-1 medication because of bone-density concerns?
Talk to your doctor — emphatically not a supplement article. These medications have substantial documented benefits, and the bone-density question is being actively researched, not treated as a reason to stop. Never discontinue a prescription based on something you read online, including here.
Does resistance training protect bone density during weight loss?
Both the mechanistic reasoning and clinical guidance point that way — bone responds to load. The right exercise plan for you should come from a clinician or a qualified trainer who knows your history.
What tests might my doctor consider?
Commonly discussed in this context are serum 25-hydroxyvitamin D, serum calcium and parathyroid hormone, with DEXA as the imaging method for bone density itself. What's appropriate depends entirely on your risk profile and your doctor's judgement.
Should I take a vitamin D and K2 supplement while on a GLP-1?
Ask your prescriber. Vitamin D status is measurable with a blood test, dosing should be based on that result, and more is not better. This is not a question a supplement brand should be answering for you.
I'm postmenopausal and starting a GLP-1 — is bone health a bigger priority for me?
Postmenopausal status is a recognised independent risk factor for bone loss, and it's a reasonable thing to raise at your appointment. Your endocrinologist or primary care physician can decide about baseline imaging, dietary input and anything else based on your specific picture.
